Application for Admission to the English Language Institute

If you send the application via our online form, you will still need to fax or mail the financial statement and sponsorship agreement (PDF download, 124kb) (required if someone else will be sending payment) and the $65.00 application fee to 1-801-585-9449. Visa, Mastercard, American Express or Discover Card information may be faxed with this application. Bank drafts, international money orders, and checks in US dollars drawn on a US bank are also accepted.
